Aging is a complex phenomenon combining biological or intrinsic aging with the influence of extrinsic environmental factors, the most important of which is probably the effect of solar UV. The end result is degradation of the extracellular matrix with poorly arranged collagen fibers, elastic fibers that have lost their elasticity, and a ground substance that is less lubricating than it was. The epidermis tends to thin out, with less active cellularity, a disorganized stratum corneum, and flattened rete ridges.

There are some cellular targets in the epidermis, such as the basal layer mother keratinocytes, Merkel cells, and the dendritic cells, namely, melanocytes and Langerhans cells. These cells are extremely important as far as keeping the epidermis healthy and happy is concerned. The major cells of interest for wound healing and rejuvenation are located in the dermal extracellular matrix, namely, the fibroblasts, mast cells, neutrophils, and macrophages. For them, neither the green nor the yellow wavelengths will penetrate deeply enough. In the case of near-infrared light, cell membranes are more or less opaque at this waveband and so the incoming energy is fully absorbed in the cellular membrane. The cellular transport mechanisms are instantly activated, and the mitochondria are prodded into action to produce more ATP to fuel this sudden cellular activity, with LED Light Therapy.

Yes, red light therapy is often referred to as near-infrared (NIR) therapy. Also known as, LLLT – Low Level Light Therapy, NIR refers to the wavelength of light that is closest to the infrared spectrum, typically between 600-1000 nanometers. Red light occupies the “long end” of the visible spectrum with wavelengths of 630nm-700nm, falls within this range and is considered a form of NIR therapy and has a penetration depth of 0.5cm. Infrared light is invisible and is effective for use on the surface of the skin as well as penetration of about 1.5 inches into the body. Infrared sits right next to red light on the electromagnetic spectrum at 800nm to 1000 nm.

Melasma, whether brown, grey or bluish pigmentation in the skin, is a condition that can have multiple factors that will complete with maintaining clear homeostasis. Those factors can be genetic and combined with the following trigger: Sun exposure direct or indirect on areas of the body that do not have melasma, hormonal changes, or pregnancy. Melanin is essential to preventing skin damage, especially damage caused by ultraviolet (UV) light, however, it can stimulate the production of melsama which is fed by melanocytes which are carried by blood closely but beneath the stratum basale layer. When exposed to UV radiation light, even short term, melanin production can result increased melasma. Using light energy is absorbed by the cells in the skin, which triggers a cascade of reactions that lead to the production of collagen and elastin.

A study published in the journal “Dermatology and Therapy” in 2023 found that pulsed PBM treatment was effective in reducing the appearance of melasma in a small group of patients. The study participants received eight weeks of pulsed PBM treatment, and the researchers found that the treatment led to a significant improvement in the Melasma Area and Severity Index (MASI) score, which is a measure of the severity of melasma.
