Overview

Staff and Travel Home Health LPNs/LVNs need a diverse skill set to succeed in their role. Firstly, they must possess strong clinical skills, including medication administration, wound care and monitoring patients' vital signs. They should be proficient in using medical equipment commonly used in home health settings, such as blood pressure monitors and glucose meters.

Home Health LPNs/LVNs must be knowledgeable about various medical conditions and their treatment plans. They should be skilled in assessing patients' needs, identifying any changes in their health status and communicating these observations to the appropriate healthcare professionals.

They also monitor patients' conditions and collaborate with registered nurses and physicians to adjust care plans as necessary.
Another critical aspect of their role is educating patients and their families about self-care techniques, medication management and disease prevention strategies. By providing education and support, Home Health LPNs/LVNs empower patients to take an active role in their own health and recovery.
